As a Parent/Guardian of the child registered in the Basketball Skills Clinic with Coach Eric Altamirano hosted by Homegrown:

I give my consent for my child to participate in the Basketball Skills Clinic on October 9 and/or October 10, 2024. I understand that my child will engage in physical activities and may be exposed to potential risks associated with the sport, including but not limited to minor injuries, falls, and collisions.

I acknowledge that it is my responsibility to ensure that my child is physically fit and healthy to participate in the sporting activity. I understand that the organizers and coaches will take reasonable precautions to ensure the safety and well-being of all participants, but accidents can still occur.

I authorize the organizers and coaches to provide necessary first aid and emergency medical treatment to my child in the event of an injury or illness during the clinic. I understand that every effort will be made to contact me in case of an emergency.

I agree to comply with the appropriate channels and processes in line with Homegrown and Sportscover insurance policies to lodge any personal claims arising out of or related to my child’s participation in the clinic.

I grant permission for my child’s photograph or video to be taken during the clinic for promotional purposes, including but not limited to websites, social media platforms, and printed materials.
